Abstract   To evaluate the effectiveness of radiation therapy and chemotherapy as a postoperative adjuvant treatment, 64 patients with locally advanced breast cancer treated with radiation theraPy alone or chemotherapy plus radiation following mastectomy between January 19SO and December 1985 were retrospectively analysed. Chemotherapy consisting of 3 cycles or more of cyclophosphamide, meth- otrexate and 5-fluorouracil was performed prior to radiation. Radiation therapy was given to entire chest wall and regional lymph nodes. The median follow-up was 46 months. The 5 year survival and relapse free survival (RFS) for all patients was 42.9% and 37.3%, respectively. The 5 year survival and RFS for patients with stage IIIA was 88.2% and 76.5% retrospectively who received chemotherapy plus radiation, and for those who received radiation alone, 33.8%. and 32.6%, retrospectively. The addition of chemotherapy showed significant improvement of 5 year survival (p<0 05) and RFS (p<0.05). However, there was no statistical differences in 5 year survival (27.5%, versus 49.4%) (p=0.1) and RFS (25.4% versus 34.3%) (p>0.1) for patients with stage IIIB. Furthermore, there was no difference in pattern of relapse between two treatment groups. Locoregional tumor control could be achieved in the majority (91.4%), while distant relapse (50%) was the major cause of treatment failure regardless of the treatment modalities. In canclusion, based on the result of this study, postoperative combined treatment of chemotherapy and radiation therapy would be beneficial to the pstients with stage II1A carcinoma of breast in terms of delaying distant metastasis and prolonging disease free survival time.
